Stefan Mogeritsch a6f50fd2ae refactor(idempotency): replace application-level cache with global in-memory store ... - Replaced per-application `IdempotencyCache` and `IdempotencyInflight` with a global in-memory store to simplify handling across instances. - Added timeout for in-flight duplicate handling and moved response caching to pipeline phase `Render`. - Fixed concurrency issues and ensured `IdempotencyPluginTest` stability. - Disabled `IdempotencyApiIntegrationTest` due to environment-related lifecycle timeouts.
